• Monday - Friday
• Salaried /Exempt – 40 hours
• Pay rate $62,000 - $70, 000/year based on experience

Requirements: Licensed Master Social Worker (LMSW)

• Licensed Master Social Worker
• Minimum 1 year Hospice or Home Health experience
• CPR certified or willing to be certified within 90 days
• Ability to utilize the latest technology for reporting and communication
• Licensed driver with automobile insurance in accordance with state requirements. Must have reliable transportation and demonstrated good driving record.
• Prepares reports for marketing activities and communication.
• Ability to work independently with a drive for success.
• Positive attitude and the ability to work as a team
• Excellent Customer Service Skills
• Excellent oral, written and interpersonal skills
• Always put the residents' needs first honoring them with dignity and respect and providing excellent care to our empowered elders.

Job Duties:  Licensed Master Social Worker (LMSW)

• Assessing the psychosocial status of individuals beginning Hospice care
• Providing ongoing follow-up as required by applicable federal and state requirements
• Developing written plans that provide information and direction for other team members
• Prepares patient social histories (e.g., cultural factors, financial concerns, and support systems) to augment existing service and to serve as a guide in determining and/or changing levels of service
